Our Process
How Does A Care Consult Work?
Here are some possible outcomes from your meeting with our Care Manager:
Needs Assessment
Any environmental safety issues, such as trip hazards, a lack of grab bars in damp places, improper usage of extension cords or space heaters, or any other safety concerns, can be found with the use of a home tour.
Review of ADLs
The care manager will then inquire about common activities of daily living, such as dressing, cooking, bathing, and getting in and out of a chair or bed. Do they need help, or are they still manageable?
Identify Opportunities for Support
Based on any gaps or shortcomings identified, the Care Manager will investigate care solutions that could help the family after evaluating the circumstances.
Home Safety Check
Any environmental safety issues, such as trip hazards, a lack of grab bars in damp places, improper usage of extension cords or space heaters, or any other safety concerns, can be found with the use of a home tour.
Social Assessment
The Care Manager will then determine whether loneliness and isolation pose any emotional or physical hazards.
Care Coordination Plan
After that, the care manager will go over any health issues, medication administration, and other matters that need to be included in a care coordination plan.
Care Consult Recommendations
Together, we will go over the findings and offer suggestions. You can talk about the kinds of care and the hours that might work best for your family.
